Who Can Register
Patients must live within the practice boundary to be eligible for registration at the practice. If you are unsure if your home address is within our boundary please use the ‘Check Postcode’ feature above.
Alternatively, you can contact the surgery on 01606 544000 and our reception team will check this for you.
Initially we will ask you to complete a health questionnaire as it is important that we have as much information as possible on your past medical history. We will, of course receive your medical records electronically from your previous practice if they offer this. If not, we will contact your previous surgery and request a summary of your records before your paper records are received which can take up to 4 weeks.
Once you have registered you will be offered a new patient health check with the Healthcare Assistant.
